First Visit
What Will Happen On My First Visit?
Please assist us at the time of your initial visit to the office by providing the following information:
- Typically, with your authorization, our office will obtain your most recent radiographs directly from your referring dentist or previous dentist for your convenience. If existing x-rays are not current (older than 1 year), Dr. Kaye and Dr. Rabe may require updated radiographs, which can be performed at our office on your first appointment.
- A thorough medical/dental history, including allergies and all medications (prescription and over-the-counter) you are currently taking.
- If you have dental insurance, please bring any forms or insurance cards with you to the appointment.
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
Please notify the office if you have a medical condition or concern at or prior to your first exam (e.g. artificial heart valves or joint replacement requiring pre-medication, uncontrolled diabetes, or hypertension).
We will complete an in-depth medical and dental health history and a thorough examination to measure for jaw/dental bone loss, loose teeth, bite, oral cancer screening, TMJ (temporomandibular joint), and other signs of periodontal disease.
